Ohio Immunization Requirements for School Attendance
All students who will be entering 12th grade are required to have Meningitis vaccine given on or after the age of 16 years of age.
All students who will be entering 7th grade next school year are required to have TDaP and Meningitis vaccines
All students who will be entering Kindergarten next school yearmust show complete immunization for DTaP, Polio, Hepatitis B, MMR and Varicella. Your child will be due for many of these immunizations after the age of 4 years.
Please make arrangements with your child’s health care provider or the Stark County Health Department 330-493-9914 ext 111 to receive the required immunizations.
Written proof of these vaccines must be turned in to the school office BEFORE the start of next school year, or your child will be excluded from attending school and all school-related activities until received.
