School Based Clinic - Forms and Information
Northwest Local Schools is working with the health department to offer free vaccines in two (2) types of clinics, depending on the students’ grade level. One clinic for Grades K-6 will be offered after school, while in-school clinics will be offered for students in Grades 7-12 at each of their individual school buildings.
