Stark County Health Department Immunization Clinic

The Stark County Health Department will be hosting, by appointment only, an immunization clinic at Northwest Middle School on Wednesday, May 15 from 3 pm - 6 pm. This Clinic is open to all children ages 0-19.

DO NO WAIT; get the school required vaccines now!  Students entering KDG, 7th and 12th grade in the fall are required to get immunizations, but all children that are need of vaccinations are welcome at the clinic.  

Please call the Stark County Health Department at 330-493-9914, extension 2050, to secure your appointment time.  

If you have questions regarding your child's immunization record, please call the school and ask for the school nurse.

Please call the Stark County Health Department for clinic details, cost, insurance questions, and scheduling.  You must have your insurance information when scheduling your appointment. If you don't have any insurance that is no problem, we can still see you.

At the time of your appointment, you will need to bring a copy of child's current shot record, your insurance card, and a parent/guardian must be present if your child is under 18.
